Proposal Application Forms:
Healthy & Fit in Southeastern Ohio
(Application period closed on October 6, 2006)

Selected public education districts in Athens, Morgan and Vinton counties were eligible to submit proposals designed to support the implementation of comprehensive and coordinated wellness plans, developed as a result of participation in the Capacity Building Phase of the Healthy & Fit Initiative. 

The Implementation Proposals are being considered in conjunction with the required documentation and successful completion of the Capacity Building Phase.
