The Osteopathic Heritage Foundations periodically seek proposals
from non-profit organizations that share the Foundations’ commitment
to improving health and quality of life. The solicited proposals
focus on mission-related, targeted initiatives. The mission of
the Foundations is advanced through a wide spectrum of grants
and funding initiatives in three broadly defined funding areas
of interest: health and quality of life, osteopathic medical
education and osteopathic research.
Generally, the Foundations do not accept unsolicited grant requests;
instead, grant requests are considered through a Request for
Proposals (RFP) process.
Funding Priority: Obesity
School Wellness Initiative: Athens, Hocking, Meigs, Morgan, Perry, Vinton counties
The goal of the School Wellness Initiative (formerly called Healthy & Fit in Southeastern Ohio Initiative) is to improve the health and quality of life of children, families, and the community through sustainable change in the areas of improved health, physical activity and nutrition, implemented through a comprehensive, coordinated plan. To accomplish this goal, the Foundation has earmarked resources to support selected local public education districts in the development and implementation of comprehensive, coordinated school wellness plans. In 2006, seven (7) local public education districts in Athens, Morgan and Vinton counties (Demonstration Districts) were invited to participate in the Initiative, and in 2007, seven (7) public schools districts from Hocking, Meigs and Perry counties (Replication Districts) were invited to participate in the Capacity Building Phase of the Initiative.
Foundation support has been designated for planning and implementation activities, designed to improve the health, nutrition, and physical activity in the community. The School Wellness Initiative is implemented in three phases: Capacity Building, Implementation Phase One and Implementation Phase Two. The Request for Proposal is available for participating districts that plan to apply for Implementation Phase One or Implementation Phase Two funds. School Wellness Grant Program: Jackson and Washington Counties
The School Wellness Grant Program is designed to improve the health,
physical activity, and nutrition of children and families in select
counties of the Osteopathic Heritage Foundation of Nelsonville (the
Foundation) service area. As one of the Foundation's strategies to
impact the obesity epidemic, the objective of the School Wellness Grant
Program is to support school district efforts to implement programs that
promote wellness, physical activity and healthy nutrition. The overall
goal of the School Wellness Grant Program is to support sustainable
change in the areas of improved health, physical activity, and
nutrition.
The Foundation will support public school districts located in Jackson
and Washington counties in implementing school wellness activities
through the provision of School Wellness Grants. The following public
school districts are eligible to participate in the School Wellness
Grant Program. In Jackson County: Jackson City Schools, Oak Hill Union
Local Schools, and Wellston City Schools; and in Washington County:
Belpre City Schools, Fort Frye Local School District, Frontier Local
Schools, Marietta City Schools, Warren Local Schools, and Wolfe Creek
Local School District.
The Foundation will host two meetings for school districts to obtain
further information regarding participation in the School Wellness Grant
Program. Attendance at the meetings is not required, but districts will
have the opportunity to ask questions, and to work on the required
materials for participation, if desired. Please RSVP your attendance to

School districts interested in participating in the School Wellness
Grant Program shall return the required documents (project narrative,
budget, budget narrative, Memorandum of Agreement, and W-9 form) to the
Foundation no later than April 30, 2008.

Grantees currently funded through the Osteopathic Heritage Foundations' Community Responsive Grantmaking Program should utilize the following report forms for their six-month and final reports to the Foundations.
